Description
It is possible to create a new page without using a link, so the new page is unlinked (OrphanedPages).
Unfortunatly the steps to create such (unwished) pages is even described here ==> HelpOnPageCreation.

Within the normal Wiki Refactoring process:
1) Weekly check pages found here OrphanedPages 2) Warn users and wait some time. 3) Delete this pages

I think at first the HelpOnPageCreation should be fixed.
Being able to do that is not a bug, but a feature.
You can create a page using a link on a page and also you can create first a page and link it afterwards. Both ways are valid. It is also valid to create pages which are not linked to any other. e.g. if you don't have a Category entry on the homepage of a user then every users homepage is orphaned. This is not a bug.
May be HelpOnPageCreation should explain orphaned pages
